The research address is for questions about clinical studies and is read by the research team. Please do not use it for appointments, prescriptions, results or referrals, since it is not monitored by clinic reception.
Referrals go by fax, not by this form
A referral must come from a physician or nurse practitioner and be faxed to 905 625 5650. Referrals or clinical images sent through the form below, or by email, are not accepted and will not be booked. Patients cannot self refer, except to enquire about a research study.

Use this form for non urgent, non clinical questions, such as a query about directions or a request for a call back. Do not include health details, health card numbers or images. We aim to respond during clinic hours on the next clinic day.

Not for emergencies
This clinic is an outpatient dermatology practice and this page is not monitored for urgent matters. If you have a rapidly spreading rash with fever, widespread blistering or skin peeling, swelling of the face, mouth or throat, or difficulty breathing, go to the nearest emergency department or call 911.
Patients already under our care who have an urgent concern about their treatment, such as a possible reaction to a medication, should telephone the clinic during clinic hours. Outside clinic hours, contact your family physician, Health811, or the emergency department.
