For family physicians

Plastic surgery at DermCare, by referral

DermCare has a plastic surgery service alongside its dermatology practice, provided by two surgeons who are Fellows of the Royal College of Surgeons of Canada. Referrals come in through the same fax as any other referral to the clinic.

  • Fax 905 625 5650 and mark the referral for plastic surgery
  • The referral is triaged to the plastic surgery service and the patient is booked for a consultation with one of the surgeons
  • A consultation report is returned to your office after the visit

Refer a Patient

  • Fax referrals to 905 625 5650 with the patient's OHIP number and reason for referral
  • Suspicious or rapidly changing lesions are triaged separately
  • A consultation report is returned to your office after the visit

On site treatment

Phototherapy

  • Covered by OHIP when ordered by a physician for a recognised diagnosis
  • Narrowband UVB for psoriasis, eczema, vitiligo and related conditions
  • Short scheduled sessions, typically two to three times a week

For Referring Physicians

How to Refer

Referrals are accepted by fax to 905 625 5650. Include demographics, OHIP number, reason for referral, relevant history and prior treatment.

Urgent Triage

Mark the referral urgent for suspected melanoma, a rapidly changing or bleeding lesion, or a severe blistering eruption.

Consultation Reports

A written report is returned to the referring physician, and to the primary care provider where these differ, after every new patient visit.
